DISABLED_APP Error Message

I'm trying to display ads in my android app, but i got this error message [i]10-31 13:22:38.123: W/SimpleAdSample(11908): Ad failed to load. Code: INTERNAL_ERROR, Message: Server Message: DISABLED_APP[/i]

I've trying to report it in "Contact Us" form, but still ddn't get any reply yet. Anyone please help me to solve how to enable an app. Here is my application key xxxx. Thanks for advance.

As for the Tax Interview: login to your Dev Portal account, click on Settings in the top navigation bar, then click on Tax Identity directly below that nav bar, and complete the form there, or alternatively try: https://developer.amazon.com/taxinterview.html
